Consent Form

I hereby request that the office of Doug Downey, Member of Provincial Parliament for Barrie - Springwater - Oro-Medonte, assist me with the following issue.

For this purpose I authorize the office of the Member of Provincial Parliament Doug Downey and/or his representative to consult the necessary provincial government departments or agencies on my behalf and for the provincial government department/agency to release any and all information in my file to the Member of Provincial Parliament and his staff.  MPP Doug Downey and his representative may use and share with government officials any and all personal information and documentation, including correspondence, which I have provided to him or his representative to the extent necessary in their efforts to assist me.

I acknowledge that all copies of correspondence, documents and the complete contents of the file complied by the office of MPP Doug Downey regarding my issue/enquiry will be destroyed by that office after each term in office. 

I understand that Member of Provincial Parliament Doug Downey and/or his representative cannot guarantee any outcome of their efforts to assist me. This consent to collect, use and share personal information as stated above will remain in effect until cancelled by me in writing, or until Doug Downey is no longer the MPP for Barrie – Springwater – Oro-Medonte.
